If this position interests you, please apply!  DESCRIPTION OF WORKAs the County Mental Health Program Specialist 1 (Behavioral Health Program Specialist) for Complex Cases, you will be responsible for assessing and coordinating the clinical care needs for adult consumers identified in two or more of the following groups: Being discharged from psychiatric inpatient or state hospitals; Admissions to Long Term Structured Residence or any other long term care facility; At-risk for multiple inpatient readmissions; Seriously Mentally Ill (SMI) consumers experiencing reoccurring acute episodes; Or a member of a priority or special needs population (i.e. Intellectually Disabled, Aging, Forensic, etc.) who present with complex behavioral health, developmental services, medical and/or other life care needs.Additionally, you will also participate in the development and evaluation of clinical guidelines or standards of care for the previously identified populations of patients.
