Westmoreland County is seeking applicants for the position of LPN Assessment Coordinator (LPNAC/LNAC) for Westmoreland Manor at the rate of $22.95 per hour!
The purpose of this position is to work in conjunction with the RN Assessment Coordinator to conduct and coordinate the development and accurate completion of the resident assessment and care plan process in compliance with all federal, state and local guidelines which govern the facility, and, as directed by the Director of Case Management, Director of Nursing and/or the Administrator so as to ensure each resident’s individual needs are identified and addressed accordingly.
Duties to include, but not limited to:
Conduct and coordinate the development and accurate completion of the resident’s MDS, entry/re-entry records and care planning process, including the CAA process for comprehensive assessments in conjunction with the RN Assessment Coordinator.
Coordinate scheduling of the MDS and care plan process following the RAI manual for MDS completion of both OBRA and Medicare/Managed Care requirements under applicable federal and state guidelines in conjunction with the RN Assessment Coordinator.
Develops and monitors care plans and the resulting impact on clinical reimbursement systems by following federal and state guidelines and producing proper diagnosis, reporting and documentation.
Attends care plan meetings.
Communicates daily with managed care case managers, providers and insurance companies regarding revenue collection.
Communicates with residents and families regarding denials and providing specific levels of care.
Reviews resident medical charts and electronic records.
Performs QA checks for follow-up care provision; maintains & updates documentation, as appropriate.
Attempts to obtain a skilled auth when needed.
Complete base line care plan for new admissions.
Evaluates, identifies and reports changes in resident conditions.
Understands and follows safety and sanitation rules, practices safety is provision of services, and promptly reports any hazards or violations to a supervisor.
Successful applicants will possess:
Ability to express ideas clearly and concisely both orally and in writing;
Ability to assemble, analyze and prepare reports;
Ability to interact effectively with the case management team and other health care disciplines.
Ability to audit and monitor nursing care for compliance with policies, procedures and standards of nursing practice, the health care facility and governmental regulations.
Education and Experience:
Graduation from a state approved school of nursing and current registration with the State Board of Nursing Examiners by examination as a Licensed Practical Nurse;
2-5 years professional experience in Long-term Care.
Proficiency in operating personal computer (excel and word) and using various types of software to input medical information
2 – 5 years experience in care planning and MDS experience required. Previous experience with PDPM, Utilization Review and/or Quality Assurance within the last 3 years is preferred.
Successful score on the Westmoreland Manor LPNAC assessment.
