Director of Case Management (RNAC) – Westmoreland Manor $6,000 Sign-On Bonus for Eligible Applicants

Westmoreland Manor is seeking applicants for the position of Director of Case Management. The purpose of this position is to monitor resident changes that effect their acuity and to oversee and complete the assessment/care plan of residents to promote their emotional and physical well-being. The Case Management Coordinator is responsible for the implementation and evaluation of effectiveness while assuring compliance with ethical standards, RAI manual, Medicare and other insurance provider’s requirements as well as Quality Initiatives. The Director will be responsible for maintaining accurate clinical records for reimbursement as well as identifying residents eligible for skilled services and obtaining insurance authorizations. Responsible for the oversight of Registered Nurse Assessment Coordinators, Licensed Nurse Coordinator, and Case Management Secretary, and working collaboratively with multiple disciplines. Compliance with all Federal and State regulations is expected. Excellent communication and computer skills required. A sensitivity to and knowledge of a geriatric population is a must. Applicants must have two to five years of MDS Coordinator/ Case Management experience in a long term care setting, including supervisory experience of a Case Management department/office. Those offered a position within the last month prior to the launch of the sign-on bonus program are not eligible. Casual, temp or part-time nurses or nurse aides are not eligible.
